LEARNING USMLE STEP 1 – HOW TO USE UWORLD EFFECTIVELY

I. PART 1 - PREPARATION PHASE

When starting to study for USMLE Step 1, one of the most popular and effective tools that most candidates use, including IMGs and even medical students in the US, is UWorld. 

Even though it is popular and famous, not everyone knows how to use UWorld effectively, and there are even some misunderstandings about how to use this tool, leading to unsatisfactory exam results.

Therefore, in today’s article, I will share my experience studying USMLE Step 1 with UWorld in an effective and smart way, helping you save years on your path to becoming a doctor in the US.

1. The Real Purpose of Using UWorld

Many people think that UWorld is a practice test, a tool to evaluate their level, and if they don’t get a high score from the beginning, they are not ready to continue studying with UWorld. 

Instead, they will learn all the background knowledge of the subjects in the test from scratch, then, when they remember all that knowledge, they will go back to taking the “UWorld practice test”.

But the truth is, you finish studying it all at once. background knowledge of all subjects like that, in the end you forget everything you just learned, also known as “learning first and forgetting later”.

This is really a huge mistake, it wastes a lot of time, money and discourages you a lot.

UWorld is a learning material practice, not just an assessment tool or a practice test! 

The important thing when using UWorld is that you should not focus on scores but should see it as a tool to help you study and review knowledge. The goal of using UWorld is to learn, not to test your level.

Don’t worry when in the beginner stage you do a block and get more than half of the questions wrong, because most people do that. Answering incorrectly is an indispensable part of the learning process. 

Give yourself the opportunity to learn from incorrect answers, read the explanations carefully to clearly understand why your answer is wrong, and why the correct answer is like that. Besides, this is also an opportunity for you to review the knowledge that you still lack. 

Over time, your background knowledge will gradually be built up, and your score will also gradually improve. But again, scores are not the deciding factor.

The important thing is that you have the opportunity to see the gaps in your knowledge, understand, and remember them, as well as get familiar with the test, learn how to handle questions, test-taking skills…

2. Read the Explanation Carefully

One of the effective secrets to using UWorld is to make full use of the explanations. 

This will be where you review detailed knowledge about the questions/topics that you answered incorrectly. Many of them will include diagrams, tables, and illustrations, helping you better understand the topic. 

Don’t try to memorize the answer to that question, because the probability of you seeing that exact question again in the real exam will be almost 0. Instead, focus on deeply understanding and remembering the knowledge in the explanation, this will help you retake questions on similar topics in the future. hybrid.

For example, when you encounter a question on the topic of diagnosing anemia, the explanation section not only provides the correct answer but also explains the pathology, treatment, and related factors of this disease. This will help you consolidate solid background knowledge, instead of just memorizing the correct answer.

UWorld has high-yield topics that will be repeated many times in different blocks (these topics are also asked over and over again in the real test).

Therefore, studying UWorld early and properly will help you a lot in remembering knowledge and succeeding in the USMLE Step 1 exam.

3. Time Mode When Doing UWorld

Anothermistake that many candidates make is not keeping a timer when doing UWorld. 

When taking the real exam, you will have to do the test in a very limited time. And if you don’t put yourself in real exam mode when studying and taking practice tests, the 7 hours of studying in the exam room will pass very quickly, leading to unsatisfactory exam results.

Therefore, when studying, use UWorld in timed mode to simulate the real exam. This will help you get used to how to manage, allocate time and mental pressure in the exam room. 

You can do such a block for an hour, then watch the explanation, learn the gaps in knowledge and continue to do the next blocks. This will not only train you to react quickly and accurately, but also help you not be distracted and maintain high concentration when doing the test.

4. Random and System Modes

Another question that I often receive from students is whether to take UWorld in Random or System mode?

This will depend on each specific exam preparation stage. For example:

When starting out, you should do UWorld system-based to create connections between questions and knowledge topics. This will also help you build connections between diseases and their mechanisms, manifestations, treatments, etc., thereby gaining a deeper understanding and longer passive memory through the spaced repetition mechanism.

After you are familiar with the systematic questions and have a certain background knowledge, you can switch to the mode random to simulate the USMLE exam question style.

5. How much time is reasonable to spend on UWorld?

The UWorld question bank has over 3,600 questions for the USMLE Step 1 exam. If you study 1 block per day (equivalent to 40 questions), you will need about 90 days to complete the first round (of course the time may vary depending on speed study your lesson). 

In the first round, the goal is to learn and understand, not just to complete a number of questions, so the learning speed may be slower. However, with your effort and effort, it will gradually improve over time. 

In addition, to prepare well for this exam, you may need to study UWorld 2-3 times to firmly grasp and remember the entire huge amount of knowledge.

II. PART 2 - THE FINAL PHASE

Once you have gone through the review period and mastered the foundational knowledge with UWorld and Board and Beyond, the exam preparation phase requires you to convert the way you use UWorld from a learning tool to a self-assessment tool.

This helps you measure your level of readiness and prepare the best strategy for the USMLE Step 1 test.

1. Why should you use UWorld as a Self-Assessment?

In this stage, the main goal is no longer to learn new knowledge but to:

  • Assess your level of readiness see if after the review process, your theoretical knowledge and test-taking skills are strong enough to achieve the target score (220+)?
  • Detect gaps in knowledge: Which concepts/systems are you weak/forgotten?
  • Build and perfect your test-taking strategy: Learn how to allocate and simulate time like the real exam day, how to handle difficult questions, maintain concentration for 8 hours…

UWorld is famous in the USMLE world as a quality question bank quality, closely following the real exam questions. Therefore, in addition to other types of self-assessment, UWorld is also an extremely suitable tool to assess capacity during this period.

2. How to Use UWorld to Assess Competencies

a. Making Full Blocks in Timed Mode

  • By default, each practice session is a real exam
  • Select timed mode and do each block of 40 questions in 1 hour, just like real exam conditions.
  • Do not stop to look at explanations or edit answers. 

b. Use Random Mode

At this stage, you should do questions in random mode to simulate how questions appear randomly in the real test. This helps you practice the ability to quickly switch between concepts and reduce the feeling of surprise when taking the exam.

c. Analyze Results in Detail

  • After completing each block, closely observe not only the percentage of correct answers but also:
    • Wrong question: Which topics did I get wrong and why? 
    • Average time: Did I spend too much time on specific questions?


d. Regularly Take Full-Length Assessments

  • Use UWorld to take a test as long as the real exam (280 questions, 7 blocks) about 4-6 weeks before the exam.
  • Create a test-taking environment similar to the real exam: start taking the test at 7:30-9am and do 7 blocks continuously within 8 hours.
  • By getting used to the real exam environment, you can evaluate your ability to maintain concentration during 8 hours of testing and get used to time pressure. Besides, you can also adjust your living, eating, and resting regimen so that you can achieve your best performance on exam day.

3. Combine UWorld with Other Self-Assessment Lessons

Although UWorld is also  a tool that is considered good and effective, you should also combine it with other assessment tests such as:

  • NBME Test Set: These are 31 practice tests that predict scores most accurately, especially recently released tests. The only downside is that you will need to pay a fee of about $50-60 for each such test.
  • Free 120: Practice test compiled  by the USMLE itself, released annually (similar to the university entrance test of the ministry of education). The exam will have the correct answer, however, there will not be a detailed explanation of the answer like UWorld.

4. When are you ready to take the exam?

One of the most frequently asked questions is: “What score do I need to score in the self-assessments to confidently take the exam?“

The answer is that you will need to score 220-230 or higher in the UWorld Self-Assessments or NBME to be able to get safe results in the Step 1 exam.

This score shows that you have mastered the basic knowledge and are able to take the test to get a good score even when affected by the pressure of the real exam.

If your practice test score is not To reach this milestone, review your learning strategy and allocate time appropriately, in addition to filling knowledge gaps and adjusting your lifestyle appropriately before performing the next self-assessment exercises.
